Purpose: This advanced care management course is to equip healthcare professionals and self home support with the specialised knowledge and skills required to effectively manage complex health conditions and provide high-quality care. By completing this course, participants will be prepared to manage intricate health conditions with a comprehensive approach, improving their ability to deliver high-quality care and support for patients facing significant health challenges.

• Duration: 9:00 AM to 4:00 PM (7 hours including breaks)
